SSA/DDS Evaluations:
What You Need to Know

If you have an evaluation scheduled with us as part of the Social Security Administration’s Department of Disability Services (DDS) process, please review the details below.

What to Expect & How to Prepare

  • Confirm your appointment: Call our office at 602-430-1458 and leave a message. Please note: we do not return calls to confirm appointments and cannot provide transportation assistance.

  • Appointment times: Appointments are set by DDS. Our office may be open on Sundays for your scheduled evaluation.

  • Duration: Consultative evaluations typically last 30–60 minutes. If psychological testing is required, please allow an additional 1–2 hours, depending on the tests requested by SSA.

  • What to bring: A valid photo ID. Bring glasses, hearing aids, or other devices if you normally use them, and take your prescribed medications.

  • Late arrivals: If you are more than 15 minutes late, you will be marked as a No Show, and you must contact your Disability Examiner to reschedule. Limited in-house reschedules may be possible at the doctor’s discretion.

  • Unable to attend or running late? Contact your Disability Examiner at (602) 771-7100 or call our office at 602-430-1458 as soon as possible.

 

Important: There is no fee for the evaluation. It may be conducted by Dr. Franza, Dr. Montijo-Tai, or Dr. Grunsten. The final decision regarding your disability claim is made by your Disability Examiner and their team.
